How To Submit an MTA

Process
You must obtain a Material Transfer Agreement for materials (biological materials, data, software or other items) for use in your research.
PLEASE DO NOT SIGN ANY INCOMING MATERIAL TRANSFER AGREEMENT OR DATA USE AGREEMENT YOURSELF. THE INDUSTRY ALLIANCES OFFICE (IAO) IS THE ONLY AUTHORISED SIGNATORY FOR UC BERKELEY TO SIGN SUCH AGREEMENTS.
- Register with the IAO. If you have not already done so, please use our MTA Registration Form. This registration allows a Principal Investigator and their designated representative (usually an Administrative Assistant or Lab Manager) to submit Material Transfer Request Forms directly through email without the need of the Principal Investigator's signature. The original signed hardcopy of this form should be sent or hand delivered to:

- Submit a MTA Request Form, available for download here. The MTA Request Form provides our office with your contact information, as well as contact information for the provider and relevant information about the material and your research. In addition, when providing the MTA itself please provide an editable version of the MTA itself so the IAO can complete the agreement. The MTA Request Form and an editable MS Word version of the MTA shall be emailed to mta@berkeley.edu.

Instructions: MTA Registration Form
Please read the instructions carefully and verify that all requested information is complete and correct.
- The MTA Registration Form requires that all fields be filled out completely.
- The Principal Investigator and, if applicable, the Administrative Assistant need to sign and date the bottom of the MTA Registration Form.
- Return the signed hardcopy to the Industry Alliances Offices.

Please read the instructions carefully and verify that all requested information is complete and correct.
- The MTA Request Form can be filled out electronically (simply tab from field to field and type directly into each field). You may also print out the form and either type or legibly complete the form by hand.
- Please fill out each section completely and remember that the “Alternate Contact” person should be someone who can both answer questions pertaining to the MTA itself and can reach the PI for signature.
- You may download the MTA Request Form in either [Word] or [PDF] format.
Note: Projects involving the use of human subjects, vertebrate animals or custom antibodies, hazardous or controlled substances, or certain other materials must have the approval of the appropriate campus committee:
- MTAs which use live animals or custom antibodies or bring them to UC must have their protocols reviewed and approved by the Animal Care and Use Committee (ACUC).
- MTAs which use human tissue or bring it to UC must have protocol reviewed by the Office for the Protection of Human Subjects (OPHS).
- MTAs which use hazardous materials or bring them to UC must follow Environmental Health and Safety (EHS) compliance procedures.
- When filling out the MTA Request Form, be sure to include the correct ACUC, OPHS or EHS protocol information.
- The MTA Request Form must be signed by you as the Principal Investigator, if you have not registered with the IAO.
